Eligibility Requirements (ODSP Guidelines)
- Be 18 years or older.
- Live in Ontario.
- Have a significant physical or mental disability expected to last a year or more, affecting your ability to work and care for yourself.
How to get approved for ODSP ?
ODSP SIGN UP PROCESS
STEP 1: Sign Up for ODSP
- Online (Apply Now)
- Over the phone by contacting the Social Assistance Contact Centre Toll-free: 1-888-999-1142 or Toll-free TTY: 1-800-387-5559.
- in-person by booking an appointment through your local ODSP office.
*The application process may take a few months.
STEP 2: Request the Mandatory Special Necessities (MSN) Benefit Form
Once approved for ODSP, ask your caseworker for the Mandatory Special Necessities Benefit (MSN) form. This form is required to get coverage for incontinence products like free adult diapers, protective underwear, and pads.
STEP 3: Visit Your Doctor or Healthcare Provider
Make an appointment with your doctor, nurse practitioner, or another healthcare professional. Explain your incontinence needs and have them fill out the MSN form to confirm your medical requirement for incontinence products.
STEP 4: Submit the MSN Form to Your ODSP Caseworker
Before submitting, double-check that all information on the form is correct, including the quantity of adult diapers or other supplies needed. Then, send the completed MSN form to your ODSP caseworker. They may also request price quotes for your products.
STEP 5: Get a Quote for Incontinence Supplies
Contact Do Home Healthcare to get a price quote for your monthly incontinence products. Be ready to provide:
- Personal Information: Name, address, phone number, and email.
- Caseworker Information: Their name, contact details, and ODSP office location.
- MID Number: Your ODSP Member ID.
- Product Details: The brand, size, and quantity of free adult diapers or other incontinence supplies you require.
- Form & Quote Details: Confirm if your caseworker needs multiple vendor quotes.
- Shipping Preferences: Choose discreet shipping and set up automatic monthly deliveries if needed.
Ensure all details match the MSN form to avoid delays.
STEP 6: Approval & First Order
Your ODSP caseworker will review the quote.
Once approved, you’ll receive confirmation, and your first order can be placed.
